13 | On Thursday, July 31, 2025, Licensing Program Analysts (LPAs) Amanda Sutter and Lea Habtom met with Director Carrie Nguyen to deliver findings regarding the above allegation. Upon arrival, LPAs observed 34 napping preschool children supervised by 5 staff and 19 napping toddler children supervised by 3 staff. It was alleged that staff left daycare child unattended.
Throughout the course of the investigation, LPAs conducted interviews and received documentation.
Through interviews, LPAs learned that Child 1 (C1) was left on the playground in April 2025. On 5/20/2025, LPA Payenda Seddiqi received an Unusual Incident Report stating that Child 2 (C2) was observed unsupervised in the small toddler play yard. LPAs also learned that in April 2025, Child 3 (C3) was playing on the playground and jumped from the play structure onto the bark. The child slipped and hit their head on the cement. The facility contacted C3's parent, who sought medical care. C3's injury required that they received staples. LPAs learned that other children were jumping from the play structure as well and staff were positioned in a way where there was no supervision of the incident.
