25 | On Tuesday July 09, 2024, Licensing Program Analysts Stephanie Piring and Office Technician (OT) Yvonne Flores met with Director, Brittany Ackerson, and Regional Director Cindy Bunton for the purpose of Amending findings to a previous complaint and to discuss supervision concerns. During todays visit, LPA observed a census of 14 napping toddlers being supervised by 2 staff and 22 napping pre school age children being supervised by 2 staff. During today's visit, LPA toured the facility and observed care.
During LPAs visit on July 2, 2024, LPA observed a toddler child walk out of the classroom to the outside play yard without staff supervision. A staff member outside setting up water play for the children found the child and notified the teachers inside the classroom. Witness statements also revealed concerns with staffs lack of supervision of children in the preschool room.
Based on observation, and witness statement, Title 22 Deficiencies have been issued on the attached LIC 809-D page and a repeat violation civil penalty of $250 is being assessed. The Facility Representative was informed that this report dated 07/09/24 documents one Type A citation and must be posted for parental review for 30 consecutive days. The facility must also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in each child's file for verification. |
