On Thursday, June 20, 2024, Licensing Program Analyst (LPA) Stephanie Piring met with Facility Representative (Director) Brittany Ackerson, for the purpose of an unannounced complaint investigation to deliver findings. LPA observed 25 napping preschool age children being supervised by 3 staff.

It was alleged that staff leaves day care child soiled for an extended period. Throughout the course of the investigation, LPA conducted interviews, reviewed relevant documents, and made observations. Interviews with the child’s parent revealed the child was wearing the same diaper they were sent to the center in, that the app (ibloom) used to communicate with parents showed no record of diaper changes, and that the child developed a diaper rash. Interviews with the Director, and staff confirmed that the child’s diaper was not changed the duration of that day. The Director stated the child was taken to the bathroom three times that day but was dry, the child slept later than usual during nap time and missed the bathroom/diaper change transition after nap.

It was alleged that staff grabbed day care child in a rough manner, on July 05, 2024, LPA was at the facility for a visit unrelated to above allegations. During this visit, LPA learned of three seperate incidents that occured while LPA was present at the facllity. Two adults reported that they witnessed two seperate occassionns where a staff grabbed at least two chidlren by the arms to move them or correct their behavior.

Based on witness statements, and record review, the preponderance of evidence standard has been met; therefore, the above allegations are SUBSTANTIATED.

The Facility Representative was informed that this report dated 06/20/24 documents one Type A citation and must be posted for parental review for 30 consecutive days. The facility must also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in each child's file for verification. Exit interview was conducted and a copy of this report was given to the Facility Representative Brittany Ackerson. Notice of site was given and must remain posted for parental review for 30 days. Appeal rights were provided.
