25 | At 1:00 p.m. on Monday, April 24th, 2023, Licensing Program Analyst (LPA) Karyn Guerra and Licensing Program Manager (LPM) Seychelle De Luca, met with Director, Kapria Hebert, for the purpose of an unannounced case management inspection. A census was conducted. At 1:10 p.m., LPA observed 2 infant children, awake, with 1 staff (S1) in the main infant classroom space. LPA observed 2 infants napping in cribs in the crib area. 2 additional infants and 1 toddler option child (C1), were napping on floor mats in the crib area. Infant child 2 (C2) was observed napping on a floor mat in the crib area. It was discovered via interview that C2 is under 1 year old and is not able to climb out of a crib. LPA observed 8 toddler option children on napping floor mats supervised by 1 staff (S2) in the walled infant activity space next to the crib space. No children were observed in the toddler option classroom.
Title 22 deficiencies are cited on the subsequent pages of this report. Director acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Licensee's signature on this report acknowledges receipt of these rights. This report was reviewed with the Director, Kapria Hebert. An exit interview was conducted. A Notice of Site Visit was provided and shall remain posted for a period of 30 days. |
