At 1:00 p.m. on Wednesday, May 31st, 2023, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Kapria Hebert, for the purpose of an unannounced complaint inspection. It was alleged that staff did not follow day care child's dietary restrictions. Throughout the course of the investigation, LPA conducted interviews and received documentation. It was learned that a child in care had a documented wheat allergy and was fed a sandwich prepared by the facility. LPA observed photo documentation of child with facility provided sandwich while in care. It was stated that there was a miscommunication regarding the child's dietary restrictions, since they were provided wheat products from home. The preponderance of evidence standard has been met, and the allegation is substantiated. A title 22 deficiency is cited on the subsequent pages of this report. Appeal rights and a notice of site visit were provided and an exit interview conducted with the Director, Kapria Hebert. Notice of site visit shall remain posted for 30 days.

At 1:00 p.m. on Wednesday, May 31st, 2023, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Kapria Hebert, for the purpose of an unannounced complaint inspection. It was alleged that staff withheld lunch from day care child. Throughout the course of the investigation, LPA conducted interviews and received documentation. LPA came to learn that a child in care had a food restriction and would bring their own food from home. It was stated that child had been offered their own food from home, but refused, and was given a restricted food prepared by the facility. The facility normally provides lunch, and morning and afternoon snacks to children in care. The allegation is unsubstantiated. Although the alleged violation may have happened or is valid, the preponderance of evidence standard has not been met to fully prove or disprove that the allegation did or did not occur, therefore, it is unsubstantiated. This report was reviewed with the Director, Kapria Hebert, and a notice of site visit provided. Notice of site visit shall remain posted for 30 days.
