On Wednesday, 03/09/22, Licensing Program Analysts (LPAs) Jan Hoshida and Jeremey McClain made an unannounced visit to the facility. The purpose of the inspection is to conclude the complaint investigation of the above allegations and deliver findings. LPAs met with the Site Supervisors McKenzie Davies and Valyncia Johnson. Upon arrival, LPAs observed seven infants supervised by four staff.

The complaint alleged that child sustained bruises while in care, facility did not properly discipline child, staff spoke to child inappropriately, facility operating over capacity, facility did not meet child’s diapering needs, and staff do not screen children for illness. During the investigation, LPA Hoshida conducted observations of children, interviewed staff and pertinent parties, and reviewed documents.

LPA reviewed medical documentation from child’s visit to the emergency room on 01/07/22 which stated that child had a small bump on the side of the head, did not observe any red marks around the child’s chest, the child appeared fine and without any other suspicious marks/bruises.

Based on staff interviews, the child accidentally tripped while carrying toys and bumped their head on the table. LPA learned that children are screened during diaper changes for any marks on their body and that the child did not have any marks on their body that day.

Based staff interviews, even though staff #1 has used a loud tone, it was not deemed inappropriate or considered a violation of personal rights. Based on staff interviews regarding discipline, staff will have children take a break and will talk to the children about appropriate and safe behavior. Based on parent interviews, there were no concerns with how the staff discipline or speak to their children.

Based on parent and staff interviews and observations, there are up to four infants in the Butterfly room with one or two staff and up to four infants in the Caterpillars room with one or two staff.

Based on observations, document review, and interviews, LPA learned that children are changed every two hours, or more frequently if they have a diaper rash, parents will be informed when and if their child has a diaper rash via the on-line communication app, and staff will address those needs based on the request of the parents. LPA observed on-line communications from the center to parents regarding diaper changes every two hours.

Based on interviews and observation, LPA learned that there is a health and temperature check conducted and documented at the door when the children arrive and that if a child has signs of illness, their parents will be contacted and asked to pick them up.

Although the above alleged violations may have happened or are valid, there is not a preponderance of evidence to fully prove or disprove that the alleged violations did or did not occur; therefore, the allegations are unsubstantiated.

The report was reviewed with the Site Supervisors and an exit interview was conducted.
