On 10/22/2025 at approximately 10:00 AM, License Program Analyst (LPAs) Pa Dao Vang and Fabian Schwartz conducted a complaint investigation inspection and met with the Director Moren Haymer. It was alleged staff are operating out of ratio and did not provide adequate supervision for children in care.

On 10/16/2025 at 9:50 AM, LPA Vang observed 16 children supervised by S1 alone. At 9:56 AM, LPA Vang observed the Director opened the door to the yard. At 10:33 AM in the Early Preschool Classroom, LPA Vang observed a C2 standing on top of the stepstool and washing his hands in the restroom for approximately a minute with no staff supervision. Both staff (S3 and S4) were attending and interacting with other children in the classroom. At 10:37 AM, LPA Vang observed S1 and S2 transitioned the children into the Two’s classroom. LPA observed 3 children opening the restroom door, walking inside, as another child closed the door. LPA pointed it out to the staff. One of the staff attended to the situation to supervise the children in the restroom.

During today’s inspection at 11:22 AM, LPAs observed 11 children supervised by S4. LPAs observed C1 used the toilet and washed her hands in the sink as S4 was supervising other children in the classroom. After a few minutes, C1 walked out of the restroom into the classroom. S4 continues to engage with other children in the classroom.

Based on observations and interviews, the preponderance of evidence results in substantiating the allegations of staff are operating out of ratio and did not provide adequate supervision for children in care.

A Title 22 deficiency is cited on LIC 9099-D. Facility representative acknowledges, that for TYPE A DEFICIENCIES ONLY upon receipt, facility shall post LIC 9099-D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the License. LIC9224 and Appeal Rights were provided. An exit interview was conducted with Director Moren Haymer s and a Notice of Site Visit posted. Failure to comply with posting requirements can result in a $100 penalty.

Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child shall be left without the supervision of a teacher at any time...Supervision shall include visual observation.

The Director will provide a copy of the training agenda to LPA on 10/16/2025. The Director will be talking to the Rosa Tichauer about a plan to prevent the lack of supervion in the restrooms. Director will email LPA Vang a plan moving forward.

This requirement is not met as evidenced by: LPAs observing children using the restroom alone on two different inspection days with a lack of supervision. This is an immediate health and safety risk to children in care.

LPA Vang leaned from the staff interviews that the facility has been operating out of ratio in more than one occasion. LPA Vang also witnessed S1 supervising 16 children in the yard for 6 minutes. This is an immediate health and safety risk to children in care.

Director have spoken to staff that when she's coming close to 11 children, then she needs to call for additional support to be in ratio. Director will email a list of staffing in the classrooms to support the ratio.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
