On 11/4/2025 at approximately 10:00 AM, License Program Analyst (LPAs) Pa Dao Vang and Fabian Schwartz conducted a complaint investigation and met with Director Mornen Haymer. It was alleged that the staff did not ensure that the facility is kept clean, and staff did not prevent day-care children from consuming food off the floor.

On 10/16/2025, 10/22/2025, and 11/4/2025, LPAs made observations, conducted interviews, gathered documentation, and reviewed files. Throughout these days, LPAs observed several classrooms and restrooms with no trash lids on them. The trash bins in the restrooms have one to several dirty diapers inside of it. On 10/16/2025, LPA Vang observed a yellow bucket with food next to a diaper trash bin with no lid in the Two’s classroom. LPA also observed the floor under the tables with gray and black streaks of residues. There was also a step stool in the restroom with gray and black streaks of residues all over it. During today’s inspection in the Early Preschool classroom, LPA Vang observed a trash bin with no lid in the restroom. There was a dirty diaper inside of it. Through multiple inspections, LPAs observed left over food in a bucket accessable to children, and diaper bins with no trash lids. LPA observed feces on the toilet seat on 11/4/2025 and residues on a step stool and floors of the Two's classroom. This is an immediate health and safety risk to children in care.

In the staff interviews, S7 stated that the leftover food is placed in the yellow bin. She also stated that in the Two’s classroom, the staff puts it on the floor during the whole lunch period accessible to children. The Director stated that they have an onsite housekeeper and janitorial services every night. Most of the staff’s interviews stated that the facility lost the funding for the janitorial services and hired a housekeeper. On S5’s interview, it was stated that the housekeeper cleans the classroom once a week but there will be trash not taken out for days. She also stated that in the Two’s classroom, they only have a broom to sweep the pasta from the floor. She stated that if there are food was left over on the floor during nap time, the children would eat it. The children’s and parent’s interviews revealed no concerns about the above allegations.

Based on observations and interviews, the preponderance of evidence results in substantiating the allegations of the staff did not ensure that the facility is kept clean and staff did not prevent day-care children from consuming food off the floor.

The Title 22 deficiencies are being cited on LIC9099-D pages. Facility representative acknowledges, that for TYPE A DEFICIENCIES ONLY upon receipt, facility shall post LIC9099-D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the License. LIC9224 and Appeal Rights were provided. An exit interview was conducted with Director Moren Haymer s and a Notice of Site Visit posted. Failure to comply with posting requirements can result in a $100 penalty.
