On 8/1/2024 at approximately 11:45a.m., License Program Analyst (LPA) Pa Dao Vang conducted a case management investigation inspection and met with the Director Yvette Pimentel.

At 12:00p.m., LPA observed 8 toddlers in the Toddler Component room in care supervised by 1 fully qualified staff member. Director explained that she has moved one of the staff in the toddler room to help the preschool room today. She also thought the toddler ratio was 12 children to fully qualified teacher. She was not aware of the toddler ratio of 1 adult to 6 children in care.

Based on LPA observations, interview conducted and record reviews, a deficiency is being cited on page LIC809-D in accordance to the California Code of Regulations, Title 22. LPA informed the Director Yvette Pimentel that this report dated August 1, 2024, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA informed the Director Yvette Pimentel to provide a copy of this licensing report dated August 1, 2024, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

This report was reviewed with Director Yvette Pimentel. A notice of site visit was provided to be posted for 30 days. Appeal rights provided.
