Licensing Program Analyst (LPA) Soleil Marx met with Facility Representative Alexis Castaneda to deliver the findings related to the above allegations. The purpose of today’s inspection was explained.

Throughout the investigation, LPA made observations, reviewed records, and conducted interviews with staff, children, and parents/guardians. Based on these efforts, it was determined that toileting procedures were inadequately supervised, as it was observed that one staff member was responsible for monitoring two separate restrooms at the same time designated for boys and girls. Additionally, through record review and interviews, it was determined that the lack of adequate supervision during toileting resulted in a child’s undergarments being put on incorrectly on at least one occasion. It was further revealed that, on at least one occasion, a child was sent home in soiled clothing. In an attempt to address these incidents, the facility sent a photo of a child wearing the correct undergarment/pull-up to a parent/guardian, in response to a parent request, with the intent of being informative. The facility acknowledged that this action was inappropriate, and the photo was subsequently removed from the communication application.

Based on observation, record review, and interview, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated.

Facility Representative was informed that this report dated 04/07/2026 documents two Type A citations which shall be posted for 30 consecutive days. Facility Representative shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Throughout the investigation, the LPA conducted observations, reviewed records, and interviewed staff, children, and parents/guardians.

Based on the information obtained, there was insufficient evidence to support the above allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations, therefore the allegations are unsubstantiated.
