At 1:40 p.m. on Thursday, May 4th, 2023, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Katherin (Katie) Wright, for the purpose of an unannounced required - 1 year inspection. Hours of operation are 7:30 a.m.- 5:30 p.m, Monday thru Friday. LPA observed a census of 54 children supervised by 7 staff. At 1:50 p.m. during census, LPA observed 3 napping toddler option children commingled with napping preschool children in the gym room.

One staff (S1) present today does not have criminal record clearance. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A health and safety inspection was conducted in the classrooms, restrooms, food service areas, and outdoor play areas. LPA observed the following documents are posted: License, Emergency Disaster Plan, Parents' Rights Poster, menus, and daily schedule. Cleaning disinfectants and hazardous items are appropriately stored and inaccessible to children. Medications are stored in the kitchen, inaccessible to children. Director stated there are no poisons on the premises. Furniture and equipment are in good condition, and toileting facilities are in safe, and sanitary condition. One urinal and one sink are in need of repair. Bins for solid waste have tight fitting lids. The floors appeared clean throughout the facility. The facility provides morning and afternoon snack. The food preparation space is free of litter and all food was protected against contamination. Drinking water was readily available to children both indoors and outdoors via water coolers and labeled bottles. Facility uses an online application for sign in and sign out. LPA observed full legal signatures of authorized representatives. There are no firearms nor bodies of water on the premises. LPA observed a carbon monoxide detector. Battery needs to be replaced in carbon monoxide detector. Playground equipment and surfaces are free of loose or sharp parts. LPA observed rubber

Staff files were reviewed. At least one staff member present today has current Pediatric CPR and First Aid certification. LPA observed immunization records and documentation of the educational background, training, and/or experience and AB 1207 Mandated Reporter training certificates.

Children's records were reviewed. Each child's file contained an emergency card, consent for emergency medical treatment and notifications of children’s and parent’s rights, health history, physician's report and immunization records. Napping logs were observed for toddler option children.

This facility provides Incidental Medical Services – IMS. LPA discussed plan of operation. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s personnel and administrative records. Title 22 deficiencies are cited on the subsequent pages of this report. Director acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Director's signature on this report acknowledges receipt of these rights. This report was reviewed with the Director, Katie Wright. An exit interview was conducted. A Notice of Site Visit was provided and shall remain posted for a period of 30 days.
