On March 21, 2024, Licensing Program Analyst (LPA) Kyrsten Williams met with director, Osiris Sanchez to open and deviler complaint findings for the above allegation. The purpose of today's inspection was explained. Present today was 46 children in care supervised by 11 staff members.

Throughout the investigation, LPA conducted interviews and made observations. LPA spoke with director and the facility's regional manager regarding cleaning schedule for the facility. Director stated the facility currently has a contracted cleaner who comes to clean the facility daily after school. The regional manager stated at times cleaning concerns have not been addressed. The facility has decided to switch to a new cleaning contactor, which will begin March 21, 2024. The director stated she cleans the carpets every friday and the teachers will clean as needed between activities. During walk through of the facility, LPA observed one carpet which appeared to be dirty.

Based on the information gathered the department has found the allegation staff does not ensure floors are kept safe, clean, and sanitary for children in care to be SUBSTANTIATED: meaning that the
allegation is valid because the preponderance of the evidence standard has been met.

As a result of the substantiated allegations, deficiencies are cited on the subsequent page of this report (LIC809-D) under the California Code of Regulations, Title 22.

The facility representative was provided a copy of their Appeal Rights (LIC9058) and the facility representatives signature on this form acknowledges receipt of these rights. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Facility Representatives, director Osiris Sanchez Palma. A copy of this report was provided.
