25 | On Thursday, May 26 2022, at approximately 8:00 AM, Licensing Program Analyst (LPA) Josiah Gathing met with Applicant Afsaneh Taghi Khari for the purpose of conducting an announced change of location pre-licensing inspection. During today's inspection, the applicant and her minor child were present in the home. All adults living in the home have received criminal record background clearance. Applicant plans to operate Sunday – Saturday from 12:00 AM to 11:59 PM.
A health and safety inspection was conducted inside and out of the home. The single-story facility includes 4 bedrooms, 2 bathrooms, kitchen, living room, breakfast nook, laundry room, garage, patio, and a fenced back yard. Off-limits areas: Bedroom 1, Bedroom 2, Bedroom 3, Bathroom 1, Laundry Room, and Garage.
There is an electric fire place in the facility covered by a glass screen. Toxic and hazardous items are inaccessible to children and out of children's reach. Sharp knives and medications are stored in the kitchen out of children's reach. A first aid kit, smoke detectors, carbon monoxide detector, and a full 2A10BC fire extinguisher were observed in the home. Required documents were posted near the facility entrance.
Applicant completed the Preventative Health and Safety course including the Lead Poisoning Prevention training on 09/22/2021. Applicant has a current EMSA certified CPR and First Aid certificate which expires 12/19/2023. Applicant stated there are no weapons in the home and there are no bodies of water on the premises. Applicant understands that prior to making alterations or additions to the home or grounds, she shall notify the Department of the proposed changes.
