On Thursday, August 18, 2022 at 10:30 AM, Licensing Program Analyst (LPA) Amanda Sutter and Office Technician (OT) Loveleen Arora met with Applicant, Josette Davis for the purpose of an announced change of ownership pre-licensing inspection. This is a change of ownership from Josette Davis as an individual to an LLC (343623192). Applicant request a preschool license to serve 40 preschool children from age two to entry into Kindergarten. Census included 25 preschool children, four staff, and Licensee. The program will operate Monday through Friday, 7:00 AM to 5:30 AM. The fire clearance was granted on 7/26/2022.

LPA and OT reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. Applicant stated the facility will provide breakfast, lunch, and a PM snack.

INDOOR ACTIVITY SPACE:
There is one classroom. LPA and OT observed sufficient amount of age-appropriate equipment, toys, tables, and cubbies. There is a first aid kit in the classroom. LPA observed medications stored in a locked cabinet above the sink. Applicant stated there are no firearms or weapon on the premises. LPA observed children using personal water bottles, which applicant stated are filled by the sink . LPA and OT observed functioning carbon monoxide detector. LPA and OT observed a paper sign-in/sign-out sheet.

LPA and OT measured the classroom. The total classroom space contains a total of 1459 square feet, which will accommodate the requested capacity for 40 preschool children. There are 4 toilets and 3 sinks available for children to use in the facility. There is a separate private restroom for the staff.

Children who become ill during the day will be isolated in the staff office or in corner near staff office. Individual measurements are recorded on the Capacity Worksheet.

OUTDOOR ACTIVITY SPACE:
There is one outdoor activity spaces for preschool children. The outdoor play area is surrounded by a wrought iron fence that is at least four feet tall. LPA and OT observed overhead shade structure and trees, that can be utilized for shade. The playground contains bark that is used for cushioning. LPA and OT observed sufficient amount of age appropriate equipment and toys.

LPA and OT measured the outdoor space. The outdoor play area contains a total of 8024 square feet, which will accommodate Applicant's request for 40 preschool children. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

LPA and OT discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA and AS discussed with Applicant any changes that may occur regarding the Director or an employee acting in the Director's absence must be reported to department within 10 working days.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
