25 | On September 13, 2023, Licensing Program Analyst (LPA) Amanda Sutter met with Director Jamie Tulba for the purpose of an unannounced Case Management inspection. LPA observed 65 children supervised by 11 staff.
On August 30, 2023, Director submitted an Unusual Incident Report (UIR) that stated that a child (C1) wandered away from the facility on 8/29/2023. LPA conducted interviews with staff involved and learned that Staff 1 (S1) was providing care for 8 children in room 132. The incident occurred around 5:14 PM, which is pick-up time. S1 said that at the same time, a child had an accident that required her to assist the child in the restroom. S1 believes the C1 left the classroom at this time.
Director provided LPA photos of the lobby area between 5:14 PM and 5:16 PM. At 5:15 PM, C1 exits through the door connecting the hallway to the lobby. A parent is holding the door open and facing away from the child. Director stated that another parent pushed the front door (between the lobby and the alleyway) open forcefully and the child had enough time to walk through quickly before it closed. Director stated that the child was found by a parent and then stayed outside with Staff 2 (S2).
LPA spoke with S2, who stated that she had clocked out at 5 PM and was leaving the facility. While walking to her car, a parent stopped her and stated that C1 had run outside of the facility and that C1’s parents were inside. S2 is not a teacher in C1’s classroom and did not recognize the child. S2 stated that C1 was found in the alleyway walking in the direction of Phoebe Hearst Elementary. S1 was able to direct C1 back to the sidewalk area. C1’s guardian then arrived at the facility.
