On 7/30/2025 at approximately 10:30 a.m., License Program Analyst (LPAs) Pa Dao Vang and Mandie Goodwin conducted a complaint investigation inspection and met with the Director Jennifer De Dios. It was alleged staff made inappropriate comments, staff did not ensure adequate food services were provided to day care children, and staff did not assist day care children with their toileting needs.

During today’s inspection at 11:34 AM, LPA Vang observed 8 preschool children supervised by S1 outside in the preschool yard. S1 was on the phone as she was yelling and using profanity in front of the children. LPA learned that S1 was on the phone with Licensee. LPAs observed the kitchen with some food in the cabinets, fridge, and freezer. LPA Vang observed two bowls of spaghetti on the countertop covered with lids in the kitchen. One of the parent Interview stated that on one occasion, a parent had donated food to the facility due to concerns of inadequate food. Another parent stated that children only had eggs for breakfast.

The Director stated there’s at least 4 different parents that have donated food to the facility because they were short on food. LPA Vang learned that Licensee orders the food every 2 to 3 weeks. The last food order was about 3 weeks ago. According to S1, she has been the only staff supervising preschool children and changing children’s diapers in the bathroom. S1 also stated that there is no other help from other staff. In two parent interviews, they stated that their children’s diapering needs were not met. One of the parents specified that there’s multiple diaper rashes. The other parent stated that she’s picked up her child with a full diaper of bowl movement.

Based on observations and interviews, the preponderance of evidence results in substantiating the allegations of staff made inappropriate comments, staff did not ensure adequate food services were provided to day care children, and staff did not assist day care children with their toileting needs.

A Title 22 deficiency is cited on LIC 9099-D. Facility representative acknowledges, that for TYPE A DEFICIENCIES ONLY upon receipt, facility shall post LIC 9099-D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the License. LIC9224 and Appeal Rights were provided. An exit interview was conducted with Director Jennifer De Dios and a Notice of Site Visit posted. Failure to comply with posting requirements can result in a $100 penalty.

Based on observation and interviews, the licensee did not comply with the section cited above when LPA witnessed staff using profanity infront of children and mutiple parents stating that their children's diapering needs are not met. This poses an immediate health, safety or personal rights risk to persons in care.
