On February 26, 2025, License Program Analyst Pa Dao Vang met with Director Geraldine Warr to follow up on the Plan of Corrections (POCs) that were cited on 1/23/2025 regarding the carbon monoxide detector, a rusted nail exposed through composing wood on the corner of the wooden platform around the tree, and a small ditch between the cement walkway into the middle of the preschool play yard. There were 7 children supervised by 1 staff.

The facility emailed LPA Vang images of the POCs to LPA Vang on 2/25/2025. During inspection, LPA Vang observed the facility corrected the POCs of removing the wooden platform around the tree, and filled in the small ditch in the middle of the preschool play yard during inspection. LPA Vang cleared the POCs as of today.

However, LPA Vang also observed left over the cement hole on the right side of the tree and a protruding nail from another small cement/wood on the ground. The Director explained that she'll cover the hole with dirt and remove the nail with a hammer tomorrow. There is also a rectangle cement protruding upward from the right side of the tree from the ground with a description on it. According to the staff, they had to stand on top of it to when the preschoolers are outside. Staff also explained that children are tripping on it a lot.

There is a Technical Violation issued on the LIC9102 page. This report was reviewed with Licensee Gealdine Warr. A copy of this report, appeal rights, and a notice of site visit was provided to be posted for 30 consecutive days.
