On 6/17/2025 at 9:15 AM, License Program Analyst (LPA) Pa Dao Vang conducted an unannounced complaint investigation inspection made to the regional office on 6/2/2025. LPA met with the facility representative, Jennifer De Dois, regarding the above allegations that the staff left infants alone without supervision and unqualified staff are supervising infants.

On 6/9/2025, LPA Vang observed a staff walking out of the facility leaving 3 infants in the room alone. LPA issued a Type A deficiency for absence of supervision.

During today's inspectionm, LPA made observations, conducted interviews, and review staff’s files. LPA reviewed S2’s course work transcripts, in which is missing the infant course to a fully qualified infant/toddler teacher. At 9:45 AM to 11:30 AM, LPA observed S2 supervising 4 infants in the infant classroom alone. The parent’s interviews show no other concerns in the facility.

On 6/25/2025 at 9:15 AM, License Program Analyst (LPA) Pa Dao Vang conducted an unannounced complaint investigation inspection made to the regional office on 6/2/2025. LPA met with the facility representative, Jennifer De Dois, regarding the above allegation that the staff are operating out of ratio. Upon arrival, LPA observed 4 children supervised by 1 staff member.

On 6/6/2025 and today's inspection LPA made observation, conducted interviews, and reviewed files. LPA witnessed 1 staff supervising children in the infant room with proper ratio. LPA also witnessed staff serving lunch, feeding an infant a bottle, and putting them to sleep. The children and parent’s interviews show no concerns in the facility. Based on LPA’s investigation, there are no Title 22 violations at the facility.

Therefore, although the alleged violations may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that they did or did not occur, therefore they are unsubstantiated. There is no deficiency issued today. An exit interview was conducted, notice of site visit, and appeal rights were provided to facility representative, Jennifer De Dois. Notice of site visit will be posted for 30 consecutive days.

Based on LPA’s observations, interviews, and file reviews, there is a deficiency being cited in accordance with California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC9099-D page. Facility representative, Jennifer De Dois, acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099-D page with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the Facility representative. Facility representative’s signature on this report acknowledges receipt of these rights. This report was reviewed with Facility representative, Jennifer De Dois. A copy of this report, appeal rights, LIC9224, and a notice of site visit was provided to be posted for 30 consecutive days.

101416.2 Infant Care Teacher Qualifications and Duties
(c) To be a fully qualified infant care teacher, a teacher shall have the following:
(1) Completion, with passing grades, of 12 postsecondary semester or equivalent quarter units in early childhood or child development education at an accredited or approved college or university.
(A) At least three of the units required in (c)(1) above shall be related to the care of infants or shall contain instruction specific to infants.

Based on the observations, interviews, and record reviews, the licensee did not comply with the section cited above as S2 is missing the curriculum course work to be a fully qualified infant teacher, which posed a potential health, safety or personal rights risk to persons in care.
