On 01/08/2024 at approximately 9:00am, Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Arianna Manabat and Licensing Program Analyst (LPA) Mandie Goodwin met with Owner Krystal Barlatt the purpose of a increase in capacity from 8 infants to 13 infants. The fire clearance (STD850) was granted and received on 12/27/2022. The program currently operates Monday through Friday from 6:00 AM to 6:00 PM

INDOOR ACTIVITY SPACE:
There is one buildings on site that is used by the facility. The infant room is located to the right of the front entrance. CAB LPA observed a sufficient amount of equipment, toys, tables, chairs, nap mats, 4 cribs, and cubbies. There are no changes to the facility's indoor and outdoor space; therefore, CAB LPA used measurements from report dated 12/16/2022. The total classroom space contains a total of 461.11 square feet, which will accommodate Applicant's request for 13 infant children. There are 4 toilets and 6 sinks for the children, and a separate private restroom for the staff. LPAs observed a changing table within arm's reach of a sink with a changing pad that is at least one inch thick with raised sides that are at least three inches tall.

OUTDOOR ACTIVITY SPACE:
There are two outdoor areas on the property, one preschool and one designated for infants. CAB LPA observed a sufficient amount of equipment and toys available for use. There are no changes to the outdoor activity space; therefore, CAB LPA did not take measurements. CAB LPA used measurements from report dated 12/27/2022. The outdoor play area contains a total of 1978.26 square feet, which will accommodate Applicant's request for 13 infant children.

The outdoor play area is fenced with a wrought iron fence that is four feet tall and separates the infant yard from the preschool yards. LPAs observed a variety of age-appropriate outdoor toys and equipment. The play structure does not include a safety label. There are no bodies of water on the premises. There are shaded areas supplied by trees. The Facility Representative was reminded to follow manufacturer guidelines for the play structure.

Licensee Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Prior to issuing a license, the following conditions must be submitted:

1. Pending Manager review by Mai Lor.
2. Furniture list

Exit interview conducted and report was reviewed with the Licensee Representative, Krystal Barlatt.
