25 | On Friday September 23, 2022, at approximately 2:30 PM, Licensing Program Analyst (LPA) Josiah Gathing met with Applicant Galyna Oman for the purpose of an announced pre-licensing inspection. Applicant plans to operate Monday – Sunday from 8:00 AM to 6:00 PM. During today’s inspection, applicant’s two adult children and two minor grandchildren were present. All individuals subject to criminal record background checks have been cleared.
A health and safety inspection was conducted inside and outside the home. The two-story home has four bedrooms, four bathrooms, kitchen, child care room, living room, sun room, garage, back yard, barn, horse arena, and four sheds. Off-limits areas include: Three bedrooms farthest from child care room, three bathrooms farthest from child care room, kitchen, living room, sun room, back yard, barn, horse arena, and all four sheds.
Sharp knives are stored in the off-limits kitchen, out of children’s reach. Cleaning supplies are stored in a cabinet in the off-limits kitchen and bathroom. LPA observed a functioning smoke detector, carbon monoxide detector, fully charged fire extinguisher, and first aid kit in the home. Fireplace is covered and gated to be inaccessible to children. Per applicant, there are no weapons or poisons in the home.
Applicant completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training on 08/20/2022. Applicant’s EMSA certified CPR and First Aid Certificate expires 07/08/2024. Mandated Reporter training is not offered in applicant’s first language, therefore she is exempt.
Applicant was encouraged to maintain supervision at all times in unfenced areas. Type A/B citations and Immediate Civil Penalty regulation deficiencies were reviewed.
