25 | On September 17, 2024 at approximately 9:30 AM, Licensing Program Analyst (LPA) Josiah Gathing met with Facility Representative Alaysia Simms, daughter of Licensee for the purpose of a required annual inspection. Facility Representative stated that the Licensee was at an appointment but would be back shortly. There were four daycare children present during today’s inspection. Days and hours of operation are Monday through Friday 6:30 AM to 5:00 PM. All individuals subject to criminal background review have obtained a criminal record clearance.
A health and safety evaluation was conducted in all areas accessible to children. Off-Limits areas: Entire second floor and bedroom, Garage on ground floor. LPA observed a fully charged 2A10BC fire extinguisher, pull alarm, first aid kit, and functioning smoke and carbon monoxide detectors. Per licensee, there are no weapons or poisons in the home. LPA observed required posted documents.
LPA reviewed a sample of children’s files and found them to be complete. LPA observed a completed Children’s Roster and fire drill log last updated August 5, 2024. Facility Representative’s CPR and First Aid certificate is valid through September 28, 2024. Facility Representative’s Mandated Reporter Certificate is valid through February 2, 2026.
