Licensing Program Analyst (LPA) Amanda Blesi met with Licensee Twonisha Bradley for the purpose of a pre-licensing inspection for a change of location. The Licensee was previously licensed under facility #343623229.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.


A health and safety inspection was conducted inside and out. The two story home has 4 bedrooms, 3 1/2 bathrooms, a living room, family room, kitchen, laundry room and garage, and an in law suite. Off limit rooms consist of all upstairs and garage. The fenced backyard is accessible for children to play. Licensee understand that 100% supervision is required in any unfenced areas.

Toxic and hazardous items are inaccessible to children. Functioning smoke and carbon monoxide detectors and a fire extinguisher size 2A10BC or larger were observed in the home. Pediatric CPR and first aid training was verified and expired on 6/2023. Licensee stated there are no weapons in the home.

Licensee has completed the required AB1207 Mandated Reporter training that will expire on 6/2023. Licensee understands that the training must be completed once every two years. Licensee understands that a current children's roster must be maintained and that a fire drill must be conducted and documented once every six months. Report Continued on LIC809-C...

Licensee understands that her license is not transferable, and once licensed, Licensee must live in the home and be present for 80% of the operating hours. LPA explained to Licensee that if she moves and wants to continue to provide care, she must submit a change of location application and have the new home inspected again.

Licensee understands that if any structural changes are made to the home; licensing must be notified prior to construction. Licensee understands that if she wants to make any off-limit areas an ON-limits for children, she must notify licensing and LPA must do an inspection BEFORE children are allowed in the areas

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual-Regulation Interpretations and Procedures for Family Child Care Homes Sections 102417. When any IMS is provided an updated Plan of Operation that includes IMS must be submitted to the department. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Exit interview conducted and report was reviewed with the licensee, Twonisha Bradley. Effective today December 12, 2022, Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Infants are children under the age of 2.
