13 | Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative, Melanie Fristoe (Representative), for the purpose of conducting an unannounced subsequent complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained. During the investigation, LPA observed the facility on 05/02/2025 and 05/23/2025 and did not observe any strong or offensive odors. Records reviews failed to provide evidence of strong odors in the facility. LPA conducted interviews with children, staff, and parents. Observations, witness statements, and document reviews did not provide corroboration that the facility was malodorous.
Although the allegation may be true or may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Melanie Fristoe. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days. |
