Licensing Program Analyst (LPA) Andrea Cortez arrived at the faciality at approximately 1:20pm and was greeted by teacher Carrie Taylor and preschool age child in the front office. The owner Briana Esquivel Diaz and Regional Director Melanie Fristoe (RM) arrived on site at approximately 1:45pm. LPA explained the purpose of conducting an unannounced complaint investigation inspection to deliver findings pertaining to the above allegation.Today's inspection was to explain to the owner and RM. During the investigation LPA conducted parent and staff interviews and found the preponderance of evidence standard has been met. Therefore, the above allegation is SUBSTANTIATED.
The owner and RM was informed that this report dated 11/19/25 documents one Type A citation and must be posted for parental review for 30 consecutive days. The facility must also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the 11/19/25 date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in each child's file for verification.
