Licensing Program Analysts (LPAs) Andrea Cortez and Fabian Schwartz arrived at the faciality and was greeted by Regional Director Melanie Fristor (RD). LPAs explained the purpose of the unannounced complaint investigation is to deliver findings pertaining to the above allegations. During the investigation LPA Cortez conducted parent, children and staff interviews, and document reviews. It was alleged that the licensee yelled in the presence of children. During three of three interviews with staff it was found that the licensee had yelled at a child (C1) during nap time in the presence of children and staff. A video was shown to the LPA Cortez confirming the Licensee yelling at the child, the licensee was heard telling the child they were done and to get out of her school. During three of three interviews with parents, it was disclosed that the licensee was seen yelling at a child who was sitting on the ground on the playground and had yelled at parents in front of their children.

It was alleged staff handled child in an inappropriate manner. Two of three interviews disclosed that the Licensee had pulled a child’s hair and backpack when the child walked away and that staff had lifted a child under their armpits above the ground on the playground.

An incident regarding a staff person pinching a child’s cheeks was also disclosed. LPAs found the preponderance of evidence standard has been met. Therefore, the above allegations are SUBSTANTIATED.

The RD was informed that this report dated 2/24/26 documents one Type A citation and must be posted for parental review for 30 consecutive days. The facility must also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the 2/24/27 date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in each child's file for verification.

Licensing Program Analysts (LPAs) Andrea Cortez and Fabian Schwartz met with Regional Director Melanie Fristor (RD), for the purpose of conducting an unannounced subsequent complaint investigation to deliver finding pertaining to the above allegation. The purpose of today's inspection was explained.

During the investigation, it was alleged that the licensee drove a transportation van through a chain link fence while children were inside. LPA conducted interviews with children, parents, and staff. Additionally, licensee provided video foorage to the department showing a black van parked while children were boarding. The video shows a chain link fenace on the ground. LPA found the interviews and video were inconstant and did not corroborate the allegation.

Although the allegation may be true or may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with RD. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
