13 | Licensing Program Analyst (LPA) Andrea Cortez and met with Regional Director Melanie Fristor (RD), for the purpose of conducting an unannounced complaint investigation to deliver findings pertaining to the above allegations. The purpose of today's inspection was explained.
During the investigation, LPA interviewed parents and reviewed documents related to the allegations. It was alleged that a child was left in soiled clothing for approximately 15 minutes, inappropriate communication with an authorized representative in the child's presence, and an uncleared staff member caring for a child while waiting the parent to arrive. The investigation revealed inconsistent interviews and video footage submitted from the licensee that did not support the allegations as a result the allegations are unsubstantiated due to insufficient evidence. Although the allegation may be true or may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with RD. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
