13 | Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative, Director Melanie Fristoe (Director), for the purpose of conducting an unannounced initial complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained. During today's inspection, LPA conducted interviews and reviewed relevant documentation. The facility filed an Unusual Incident Report (UIR) self-reporting this incident. Based on witness statements and record review, the facility staff did leave a child unsupervised on the playground for one to three minutes. The preponderance of evidence standard has been met; therefore, the above allegation is SUBSTANTIATED.
The Facility Representative was informed that this report dated 06/10/2025 documents one Type A citation and must be posted for parental review for 30 consecutive days. The facility must also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the 06/10/2025 date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in each child's file for verification.
