25 | On 10/24/2025, Licensing Program Analyst (LPA) Soleil Marx, met with Licensee, Maliha Basiri, for the purpose of conducting an unannounced annual inspection. During the inspection, LPA observed a census of four children in care being supervised by the licensee and assistant. Facility hours of operation are Monday through Friday 08:00 AM to 09:00 PM.
A health and safety inspection was conducted in the areas accessible to children. Off-Limits areas: Entire upstairs, all areas/rooms on the right side of the home, kitchen, and converted garage. Day care is operated out of left side of home including day care room, connecting living room behind the day care room, and the backyard. Off-limit areas are inaccessible by baby gates, closed/locked doors and supervision. LPA observed a working telephone, first aid supplies, functioning smoke/carbon monoxide detector, and a fully charged 2A10BC fire extinguisher. LPA observed home appeared safe, sanitary, and orderly. LPA observed hazardous items such as cleaning solutions, medication, sharp utensils, and other items that could pose a danger to children in care, are inaccessible to children in off limit areas. LPA observed fireplace has glass barrier and stairs are barricaded with a baby gate. LPA observed a variety of age-appropriate toys, equipment, and materials available to children. LPA observed personal rights and supervision requirements being met during the inspection. Licensee stated there are no poisons, firearms, or bodies of water on the premises. LPA observed an empty/drained fountain in the front yard.
LPA reviewed records for facility, Licensee/assistant, and children. LPA observed required postings were posted/visible to parent/guardians. LPA observed a children's roster maintained and a fire drill log with drills being conducted/documented every 6 months; last drill 05/2025. LPA reviewed children’s files and verified files contained the required licensing forms. LPA reviewed records of Licensee/assistant and verified files were complete with licensing forms/documentation. Licensee has current approved pediatric CPR/First Aid training which is valid until 08/2026. Licensee understands CPR/first aid training must be completed every two years. Licensee is exempt from Mandated Reporter Training.
