At 9:15 a.m. on Monday, August 7th, 2023, Licensing Program Analyst (LPA) Karyn Guerra met with Licensee, Hiresha De Silva, for the purpose of an announced, prelicensing, change of location inspection. Facility is previously licensed at 343619716. Applicant requests an preschool license to serve 145 children ages 2 to 6 (entry into first grade), and 5 toddlers age 18-36 months under a toddler option program. The program will operate Monday through Friday from 7:00 AM to 6:00 PM. A fire clearance is on file with the department.

Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, car seat law, menus, and daily schedule. LPA discussed the forms that must be in each child's and each staff member's file and provided form 311A. The facility will provide morning and afternoon snacks.

INDOOR ACTIVITY SPACE:
The indoor classroom space was measured and documented on a capacity worksheet. There is a total of 8 classrooms at the facility with a total area of 7556 square feet which will accommodate the requested capacity of 150 children. Casa 5 classroom is currently being used for storage. LPA observed a sufficient amount of tables, chairs, cubbies, and equipment for the requested capacity. Licensee stated that additional curriculum materials are being transported from the previous facility. First aid kits are located in each classroom. Licensee stated medications will be stored in classrooms, with an additional locked box in the refrigerator. Licensee stated there are no poisons nor firearms on the premises. Licensee stated that water for drinking purposes will be delivered to the facility. LPA observed carbon monoxide detectors in all classrooms. The facility will use paper sign in and sign out forms.

There is a total of 9 toilets and 3 urinals available to children at the facility. Some toilets are located within classrooms, and there are additional restroom areas in the hall corridors. One stalled toilet will be designated for staff use. Children who become ill during the day will be isolated in the staff room or office space and will use the staff toilet, if necessary. The facility has changing tables in the toddler and 2's classrooms with sinks within arms reach.

OUTDOOR ACTIVITY SPACE:
The play space is surrounded by a chain link and wooden fence that is at least four feet tall. Licensee stated that additional temporary shade will be provided by umbrellas. Licensee stated that 2 permanent canopy structures have been purchased and will be installed. There is a climbing structure that has a safety label that reads it is intended for children ages 5 to 12 years old. Licensee stated that children will not be using the structure and will remain on the ground floor. Plastic house structures and a water tables have been placed within the bark area. Additional outdoor equipment was observed. Licensee acknowledges staff must ensure children use age-appropriate equipment at all times. Additional age appropriate play structures from the previous facility are being installed. Licensee will reach out to the department when construction is complete.

The outdoor play area contains a total of 129,187.1 square feet, which accommodates the request for 150 children. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

Applicant provided a plan of operation for IMS services. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. LPA will provide PIN with additional information.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage athttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds.
LPA discussed with Applicant any changes that may occur regarding the directors or an employee acting in the director's absence must be reported to department within 10 working days. LPA discussed lead testing of water requirements AB2370. Licensee understands that lead testing of drinking and cooking water shall be conducted within 6 months of licensure. Licensee stated that water in the facility will not be used for drinking nor cooking purposes.

This facility evaluation report was reviewed and discussed with Applicant. Applicant was encouraged to the visit the Department's website at WWW.CDSS.CA.GOV for information regarding child care updates, forms, regulations and legislation pertaining to child care centers.

Effective today, August 7th, 2023, the location change is granted pending final approval from LPM De Luca.
