25 | At 9:15 a.m. on Monday, August 7th, 2023, Licensing Program Analyst (LPA) Karyn Guerra met with Licensee, Hiresha De Silva, for the purpose of an announced, prelicensing, change of location inspection. Facility is previously licensed at 343619716. Applicant requests an preschool license to serve 145 children ages 2 to 6 (entry into first grade), and 5 toddlers age 18-36 months under a toddler option program. The program will operate Monday through Friday from 7:00 AM to 6:00 PM. A fire clearance is on file with the department.
Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, car seat law, menus, and daily schedule. LPA discussed the forms that must be in each child's and each staff member's file and provided form 311A. The facility will provide morning and afternoon snacks.
INDOOR ACTIVITY SPACE:
The indoor classroom space was measured and documented on a capacity worksheet. There is a total of 8 classrooms at the facility with a total area of 7556 square feet which will accommodate the requested capacity of 150 children. Casa 5 classroom is currently being used for storage. LPA observed a sufficient amount of tables, chairs, cubbies, and equipment for the requested capacity. Licensee stated that additional curriculum materials are being transported from the previous facility. First aid kits are located in each classroom. Licensee stated medications will be stored in classrooms, with an additional locked box in the refrigerator. Licensee stated there are no poisons nor firearms on the premises. Licensee stated that water for drinking purposes will be delivered to the facility. LPA observed carbon monoxide detectors in all classrooms. The facility will use paper sign in and sign out forms.
