25 | Licensing Program Analysts (LPAs) Erwina Pascual-Golamco and Kyrsten Williams and Licensing Program Manager Natalie Dunaway met with Applicant Kothai Sadishkumar for the purpose of an announced pre-licensing (change of ownership) inspection. Also present during this inspection was co-owner Sadish Thiyagarajan. Facility was previously licensed under facility number 343622470. Applicant is requesting preschool license with a maximum of 22 preschool children. The program operates Monday through Friday from 6:30AM to 6:00PM. There were no preschool children in attendance at time of visit. The fire clearance was granted on 07/18/2023.
LPA reviewed with applicant the LIC311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule.
INDOOR ACTIVITY SPACE:
The preschool has 2 classrooms and a kitchen to support food preparation. LPAs observed sufficient age-appropriate equipment, toys, tables, chairs, cots, and individual cubbies. There is a first aid kit in each of the classrooms. Applicant stated medications will be stored in the office and inaccessible to children in care. Applicant stated there are no firearms or weapon on the premises. Water will be provided by individual water bottles and water jugs in the classrooms and the preschool play yard. LPAs observed a functional carbon monoxide detector in the facility. Applicant stated the facility will be utilizing an electronic sign in and sign out system. Applicant stated the facility will provide lunch, morning, and afternoon snacks.
