13 | Licensing Program Analyst (LPA) Gagandeep Singh met with the facility representative, Deena Nickel and Aaliyah Harris, to deliver the findings of the complaint. Upon LPA's arrival, there were 19 children in care.
During the investigation, LPA inspected the facility, reviewed records and interviewed the facility staff. During the investigation, it was found that there were multiple incidents of a child bitting other children in 'Toddler' classroom. It was found that the staff failed to kept the bitting child injuring other children in care. During the record review, LPA did not observe the acknowledgement signed by parents for accepting the Type A deficiencies cited on August 19, 2025. LPA reminded the facility representative that the copy of Type A deficiency must be provided to the parents/authorized representative within next buisness day and have form LIC 9224 signed as acknowledgement. Based on the interviews and record review, the preponderance of evidence standard has been met, therefore, the above allegations are found to be SUBSTANTIATED. See next page for deficiencies cited today. Type Acknowledgement forms are to be signed by current parent of the facility and new parents for the next twelve months. LIC 9224 and Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed and provided to the facility representative. |
