This is an amended report for the pre-licensing report that was conducted on 11/27/2023. This report reflects information for the single license. This report details information from the following pre-licensing reports: 343625306 LEARNING PATCH CHILDRENS CENTER - (INFANT LICENSE) and 343625308
LEARNING PATCH CHILDRENS CENTER - (SCHOOL AGE LICENSE). The following information is reflective of the census, information, and measurements recorded on 11/27/2023.

On 11/27/2023, Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Arianna Manabat and LPA Jennifer Velasco met with Applicant Zhuoren Ma for the purpose of an announced change of ownership pre-licensing inspection. This is a change of ownership from Learning Patch Children's CTR, THE (343621871). The Applicant requests an infant component which will serve 6 children ages 0-2 years of age, a toddler component which will serve 14 children ages 18-36 months, a preschool component which will serve 43 children ages 2 years to entry into kindergarten, and a school-age component which will serve 37 children ages enrolled into kindergarten to 12 years of age.

Today's census included 14 infant children, 25 preschool children, 0 school-aged children, seven staff, the Director, and Applicant. The program will operate 7:00 AM - 5:00 PM Monday through Friday. The fire clearance was granted on 10/10/2023. LPAs reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. Applicant stated that the facility does not provide snacks or lunch.

There is one room for the infants, one room for toddlers, three rooms for the preschool space, and one room for the school-aged components. Both LPAs observed sufficient amount of age-appropriate equipment, toys, tables, and cubbies. There is a first aid kit in each classroom with the main one in the kitchen area. Applicant stated medications are stored in the Director Office. Applicant stated there are no firearms or bodies of water on the premises. Applicant stated indoor and outdoor drinking water will be provided via water bottles and drinking fountain. LPAs observed and tested the carbon monoxide detector within the facility. LPAs observed a sign-in/sign-out sheet posted at the front entry point.

The facility's classrooms measured as a total of: 290.83 square feet for the infant classroom, 578.32 square feet for the toddler classroom, 2035.85 total square feet for the preschool classrooms, and 1454.56 total square feet for the school-aged classrooms which is enough space to accommodate for the 6 infants, 14 toddlers, 43 preschoolers, and 37 school-aged children respectively.



In the infant room there is a total of one changing table within arms reach of a sink, 6 cribs, and 2 sinks available for children's use. In the Toddler room there is one changing table within arms reach of a sink, three sinks (one which is next to the changing table) and one toilet. In the preschool room there are a total of 8 toilets and 9 sinks available. The facility is utilizing the school aged restrooms for preschoolers. LPA Manabat has requested that the facility request a shared restroom waiver to use these restrooms. Finally, in the school-aged room there are a total 4 toilets and 3 sinks available for children to use in the facility. Per child to toilet and sink ratio, the number of toilets and sinks in each room will accommodate the requested capacity for each component. There is a separate private restroom for the staff. Children who become ill during the day will be isolated in the front office area and will utilize the staff restrooms. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).

OUTDOOR ACTIVITY SPACE:
LPAs observed a total of four outdoor yards on site separated by chain-linked fencing. The infant component will utilize one of the yards, the yard adjacent to the infant room. The toddler component will utilize the one yard adjacent to the toddler room. The preschool component will use the yard that is adjacent to the parking lot. The school-aged yard is the play yard that is between the preschool and toddler yard.

The entire outdoor play area is surrounded by a chain link fence that is at least four feet tall. LPAs observed trees and canopies that can be utilized for shade. The playground contains soft floor mats for cushioning for the infants and toddlers and bark for the preschool and school age yards. LPAs observed sufficient amount of age appropriate equipment and toys. Applicant was reminded that children are to use age appropriate toys and equipment.

LPAs measured a total of 575.47 square feet in the infant yard, 2265.12 square feet in the toddler yard, 3827.80 square feet in the preschool yard, and 4356.19 square feet in the school-aged yard which will accommodate Applicant's request for 6 infants, 14 toddlers, 43 preschoolers, and 37 school-aged children respectively.

LPAs discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPAs discussed with Applicant any changes that may occur regarding the Director or an employee acting in the Director's absence must be reported to department within 10 working days.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Facility Representatives was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
