25 | On Monday, November 13, 2023, Licensing Program Analyst (LPA) Amanda Sutter met with Applicant Jenifer Arizaga for the purpose of a change of location pre-licensing inspection. Applicant was previously licensed under facility #343624322. During today’s visit, applicant’s husband was also present in the home. All individuals subject to criminal background review have obtained a criminal record clearance. Applicant plans to operate Monday through Friday from 7:00 AM to 5:30 PM.
A health and safety inspection was conducted inside and outside the home. The facility is a two-story house which includes 4 bedrooms, 2 bathrooms, kitchen, a living room, breakfast nook, backyard, and a garage. Off-limit areas will include: entire upstairs and garage. Applicant understands that children may never enter these off-limit areas.
Toxic and hazardous items are inaccessible to children and out of children’s reach. Sharp knives and medication are stored above the kitchen cabinets out of children’s reach. A functioning smoke detector, carbon monoxide detector, and 2A10BC fire extinguisher were observed. There are no bodies of water on the premises. LPA observed stairs and a fireplace barricaded according to regulation. Applicant stated that there are no weapons in the home. LPA advised the applicant that if there are ever any poisons at the home, all poisons must be locked with a key lock or combination lock. LPA discussed all required licensing postings.
The applicant provided proof of control of property. The applicant has not obtained a signed Property Owner/Landlord Consent form (LIC9149). Without this consent, the applicant understands that, once licensed, they can operate with a maximum capacity of 6 children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 8 children.
Current EMSA approved pediatric CPR/First Aid trainings were verified and expire 11/30/2024. Applicant has a current Mandated Reporter certificate which will expire 1/9/2025.
