25 | On Monday April 22, 2024, at approximately 9:00 AM, Licensing Program Analyst (LPA) Loraine Perez and Licensing Program Manager (LPM) Amanda Blesi met with Applicant Bibi Zahra Haydari for the purpose of an announced pre-licensing inspection. Applicant plans to operate Sunday- Saturday 7:00 AM to 5:00 PM. Applicant’s two adult sons and three minor children were also present during today’s inspection. Applicant’s first language is Dari and 14 year old son provided translation during today’s inspection. All persons subject to criminal record background review have been cleared.
A health and safety inspection was conducted inside and outside the home. The two story home has three bedrooms, three bathrooms, living room, dining room, kitchen, garage, covered patio, and backyard. Off-limits areas: Dining room, Kitchen, Garage, 2nd floor and sideyards. No bodies of water were observed.
Kitchen cabinets are secured with locks to prevent children’s access to sharp knives or cleaning supplies. LPA observed a functioning smoke detector, carbon monoxide detector, and full 2A10BC fire extinguisher in the home. Per applicant, there are no weapons in the home.
Applicant completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training on December 19, 2023. Applicant’s EMSA certified CPR and First Aid Certificate expires November 11, 2025. Applicant’s Mandated Reporter Training expires March 2, 2026.
Applicant was encouraged to maintain supervision at all times in unfenced areas. Type A/B citations and Immediate Civil Penalty regulation deficiencies were reviewed.
Applicant was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
