Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Arianna Manabat and Licensing Program Manager (LPM) Mai Lor met with the Licensee Representative Lisa Lytle for the purpose of an announced pre-licensing inspection. The applicant is requesting to serve 246 students; 40 infants ages 0-24 months and 206 preschool children ages two years to entry into kindergarten during the hours of Monday through Friday from 7:15am - 5:15pm (infants), and 7:00am-5:30pm (preschool component). The facility will provide morning and afternoon snacks; lunches will be brought from home, however, the facility does offer parents/guardians to purchase a lunch if they choose. LPAs reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

INDOOR ACTIVITY SPACE:
There are 13 classrooms inside the facility. Rooms 1,2,3,4,5,6,11,12, and 13 will be used for the preschool and rooms 7,8,9,and 10 will be used for the infant component. LPA observed that there are appropriate preschool supplies currently displayed in the classroom. There is a first aid kit located in each classroom above the sink area and is not accessible to children in care. Medications will also be stored atop of the cabinet. The Licensee Representative stated that medications are stored in medication boxes above the sink and any refrigerated medications will be located in the front office room. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children in the cabinet above the sink.

The Applicant stated there are no poisons, bodies of water, or firearms on the premises. LPA Manabat discussed the importance of storing any poisons in a locked area with either a combination or a key lock.

During todays visit, LPA Manabat observed a functional carbon monoxide detector in the building housing the infant component. LPA discussed the sign-in/sign-out sheet system being utilized via the QR code at the front of each door to the classrooms. LPA Manabat discussed with Lisa Lytle that the name on the sign-in sign-out sheet will need to be changed from the previous facility name to the Destiny name. Water is provided to all classrooms via water bottles and refilled using the filtered water jug that is being used. During today's visit, LPA Manabat observed that there were water faucets attached to the sinks. The facility representative stated that the children will not be using these water faucets and that all drinking water in the facility has already been lead tested.

Measurements:
LPA measured all classroom space for both the infant and preschool component. The four infant classrooms contain a total of 1,595 square feet, 4 sinks and 17 cribs designated for crib infants, which will accommodate the applicant's request for 40 infants. The nine preschool classrooms contain a total of 6,860 square feet, 18 toilets, and 18 sinks, which will not accommodate the Applicant's request for preschool children. However, during today's inspection, four of the nine preschool classrooms are ready for operation. LPA Manabat measured the four preschool classrooms (2,3,5 and 6) which contain a total of 2,993 square feet, which can only accommodate 84 preschool children.

The Licensee Representative stated that there was a separate private restroom for the staff located in the building that houses the infant component. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Children who become ill during the day will be isolated in the front office area.

OUTDOOR ACTIVITY SPACE:
There are three outdoor areas on the property, two for the preschool component and one for the infant component. Each outdoor play area is fenced with a chain linked fence that is at least four feet tall. Measurements were taken for the outdoor space footage. The outdoor play areas for the preschool contain a combined total of 15,038 square feet, which will accommodate the applicant's request for 206 preschool children. The outdoor play area for infants measures for a total of 1,510.45 which measures for approximately 20 children. This will not accommodate the Applicant's request for 40 infant children. The applicant has submitted an rotating outdoor waiver. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Report continues on LIC809-C.

PAGE 3 - There is shade provided by trees, overhangs of sides of the building, and wood gazebos. During today's inspection, LPA Manabat observed a play structure in the second preschool outdoor area with a label that stated it was appropriate for ages 2-5 years of age with enough substrate under to cushion a fall. The applicant stated that water is being provided via filtered water jugs and water bottles outdoors. There are sufficient amount of equipment and toys with additional bikes and outdoor materials stored in the shed on the second preschool playground. The Licensee Representative stated that the shed remains locked throughout the day and that staff will have access to the key. The key is being stored inaccessible to children in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPAs discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPAs discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/
safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following items are required before a license will be issued:

1. Manager review of the revised admission agreement.
2. An approved outdoor waiver
3. A new fire clearance from the fire marshal to reflect updated capacity numbers.
4. Manager review of the recently submitted operating agreement
5. Manager review of the LIC401
6. Manager review of the LIC200A.
