Licensing Program Analyst Erwina Pascual-Golamco (LPA) met with Facility Representative (FR), Katie Pack for the purpose of opening and closing a complaint investigation of the above allegation. The purpose of today's inspection was explained.

It was alleged that staff do not ensure hazards are made inaccessible to daycare children. During today's inspection, LPA observed that the cubbies in the toddler bathroom had diaper creams and over the counter ointments that were within children's reach. LPA interviews stated staff supervise the children to the bathroom and place the ointments where children cannot reach it but must have forgotten to place it back up high after using. Based on LPA inspection and observation, the preponderance of evidence standard has been met, therefore the above allegation is SUBSTANTIATED.

LPA informed Facility Representative (FR) Katie Pack, that this report dated 03/04/26 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Also, LPA informed the FR to provide a copy of this licensing report dated 03/04/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Facility Representative (FR) Katie Pack. Appeal rights provided.
