13 | Licensing Program Analyst Erwina Pascual-Golamco (LPA) met with Facility Representative (FR) Katie Pack, to deliver findings. Today's visit, LPA observed 11 preschool children with 2 staff, 6 toddlers with 1 staff and 2 infants with 1 staff.
Throughout the course of the investigation, LPA toured the facility, observed staff providing care to children, conducted interviews and requested facility documents. Staff and Director interviews revealed that ratios are maintained by keeping a name to face chart throughout the day and they have floater staff to call and step in as needed to ensure ratios. LPA’s interviews, statements and documentation were inconsistent to corroborate the allegation: Licensee does not ensure facility operates within ratio at all times. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided, exit interview conducted, and report were reviewed with Facility Representative, Katie Pack. |
