On February 19th, 2026 Licensing Program Analysts (LPAs) Mandie Goodwin and Sala Vang met with director Nicole Perez to open a complaint and deliver findings for a complaint investigation. Upon arrival LPAs observed 20 children, including 16 preschoolers and 4 babies supervised by four staff members.
During the course of the investigation LPAs made observations, collected documentation, and conducted interviews. Director stated in an interview that a mobile infant child crawled towards a wooden support post of a gazebo/sun shade in their outdoor play yard. The infant rocked forward and bumped their head on the support leaving a bump. LPA reviewed a video of the incident and observed that a staff member was outside with the children and providing supervision. The staff member was observed on the video immediately caring for the infant. LPAs toured the outdoor space and the wooden post. LPAs observed the post to be in good condition and not preventing a hazard at this time.
Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. No title 22 deficiencies are cited. Exit interview was conducted with director Nicole Perez and Notice of Site Visit was provided.
