25 | On August 27, 2024, Licensing Program Analyst (LPA) Kyrsten Williams met with Applicant, Nabras Mahmood, for the purpose of an announced pre-licensing inspection. During today’s visit, applicant’s spouse and adult daughter were present in the home. Hours of operation: Monday through Friday from 7:00 AM to 5:00 PM.
A health and safety inspection was conducted on the interior and exterior of the home. The home is a single-story house which includes three bedrooms, one bathroom, living room, playroom, second living room, kitchen, backyard, garage, and detached guest house. Off-limit area includes bedroom #1, bedroom #3, kitchen, garage, and detached guest house. Applicant acknowledges childcare children may never enter these off-limit areas. The facility has equipment and toys safe for children. Toxic and hazardous items, sharp knives, and medications are inaccessible to children. LPA observed a functioning smoke detector, carbon monoxide detector, and 2A10BC fire extinguisher within the home. LPA advised the applicant that if there are ever any poisons at the home, all poisons must be locked with a key lock or combination lock. Applicant stated there are no weapons on the premises. LPA observed a small fountain in the backyard. Applicant stated the fountain is not hooked up to water. LPA advised applicant to fill the fountain with objects to prevent excess water build up.
Applicant has completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training. Current EMSA approved pediatric CPR/First Aid training was completed and expires May 2026. Current Mandated Reporter Training was completed and expires in May 2026. Applicant understands CPR/First Aid Training and child care portion of Mandated Reporter Training must be completed every two years.
LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Applicant understands a current roster of children enrolled must be available and maintained for a period of three years, even after children are no longer attending.
