25 | On Tuesday, December 17, 2024, at approximately 2:45 PM, Licensing Program Analyst (LPA) Josiah Gathing met with Applicant Elvira Dziuba for the purpose of an announced pre-licensing inspection. Applicant’s assistant was also present during today’s inspection. Applicant’s first language is Russian and applicant’s assistant provided interpretation. Applicant plans to operate Monday – Friday 6:00 AM to 10:00 PM. All persons subject to criminal record background review have been cleared.
A health and safety inspection was conducted inside and outside the home. The single-story home has three bedrooms, two bathrooms, a living room, a kitchen, a laundry room, a garage, a back yard and a swimming pool. Off-limits areas: Bedrooms #2 and #3, Kitchen, Bathroom #2, Laundry room, Garage, Back Yard, and Swimming pool. LPA noted that the fence around the swimming pool was less than five feet high. LPA discussed upcoming swimming pool requirements with applicant.
LPA observed a functioning smoke detector, carbon monoxide detector, 2A10BC fire extinguisher, and first aid kit in the home. Per applicant, there are no weapons or poisons in the home.
Applicant completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training on March 28, 2024. Applicant’s EMSA certified CPR and First Aid Certificate expires March 19, 2026. Applicant’s Mandated Reporter training certificate will expire April 3, 2024.
Applicant was encouraged to maintain supervision at all times in unfenced areas. Type A/B citations and Immediate Civil Penalty regulation deficiencies were reviewed.
