25 | Licensing Program Analyst (LPA) Loraine Perez met with Applicant Yevgeniy Mikhalchuk for an announced return prelicensing inspection. Initial inspection was preformed on 02/05/2025. Present during the inspection is the applicant alone. The purpose of the visit is to confirm the applicant lives in the home as required for licensure. All individuals subject to criminal background review have obtained a criminal record clearance. Hours of operation will be: Monday to Friday 8 AM to 5 PM.
The home is a two-story house which includes three bedrooms and three bathrooms, a kitchen with dining area, two living rooms, laundry room, garage, and fully fenced backyard with an outdoor shed. Off-limits areas include: entire second floor, laundry room, garage and outdoor shed. Applicants understands children may never enter off-limit areas.
A health and safety inspection was conducted on the interior and exterior of the home. LPA observed personal belongings and clothing in the bedroom of the applicant. LPA observed personal hygiene products in the bathroom that belong to the applicant. LPA confirms that the applicant does live in the home.
Effective today, 02/07/2025, this facility is approved for a Large Family Child Care Home license for a max capacity (when there is an assistant present) - 12 no more that 4 infants. Capacity 14 - no more than 3 infants and 1 child in Transitional Kindergarten or above and 1 child at least age 6. Capacity without assistant present - 8 no more than 3 infants present, 1 child in Transitional Kindergarten or above and 1 child at least age 6. Infants are children under the age of 2.
