25 | On 07/14/2026 at approximately 09:00 AM, Licensing Program Analysts (LPAs) Loraine Perez and Sala Vang met with Applicant, Vera Mikhalchuk for the purpose of an announced prelicensing change of location inspection. Applicant is previously licensed at facility 343624087. Present during today’s visit were the Applicant, her husband and four minor children one of which counts toward capacity. All individuals subject to criminal background review have obtained a criminal record clearance. Hours of operation will be: Monday to Friday 7 AM to 11 PM.
The home is a two story house. On the first floor there is a primary bed and bath used for child care, living room, family room, kitchen with dinning area, laundry room, three car garage, fully fenced backyard. On the second floor there are three bedrooms and one primary bed and bath, loft, bathroom, and balcony. Off-limits areas include: laundry room, garage, entire second floor. Applicants understands children may never enter off-limit areas.
A health and safety inspection was conducted on the interior and exterior of the home. Toxic and hazardous items, sharp knives, and medications are inaccessible to children. LPA observed a functioning combination smoke and carbon monoxide detector, and a fully charged 2A10BC fire extinguisher within the home. LPA observed a fireplace and stairs are barricaded to meet regulation. Applicant stated there are no weapons or poisons at the home. LPA observed there are no bodies of water on the premises. Pets are up to date with vaccinations and will be kept with the children in care.
Applicant understands that licenses are not transferable, and once licensed, licensees must live in the home and be present for 80% of the operating hours. Applicant has current EMSA approved pediatric CPR/First Aid trainings were completed and expire 09/2027.
