25 | On May 8, 2026 at approximately 1:15 PM, Licensing Program Analyst (LPA) Stacey Williams, IB Investigator Vincent Moleski, and Regional Manager (RM) Roxana Saravia met with Licensee, Paulina Lopez for the purpose of an unannounced case management inspection to serve a Temporary Suspension Order (TSO). LPA and RM observed twelve children present in the home.
On April 8, 2026, an adult in the home sexually abused Child #1 on 3 occasions. The family of Child#1 notified the licensee of these incidents the same day, April 8, 2026. Between April 8, 2026, through May 08, 2026, the Licensee failed to report this Unusual Incident to the Department, thereby threatening the physical and emotional health and safety of children in care.
This report and the TSO documents were left with the licensee, Paulina Lopez. The following documents were included in the TSO packet: (1) Order for Temporary Suspension of Licensee Prior to Hearing, (2) Statement to Respondent, (3) Government Code Sections, (4) Instructions for Licensee about the Summary of Charges, (5) Accusation (License Revocation), and (6) Request for Discovery and Notice of Defense (two copies).
The licensee may not operate their Family Child Care Home after the close of business on May 8, 2026.
An exit interview was conducted and report was reviewed with the licensee, Paulina Lopez. Title 22 deficiencies are cited on the subsequent pages of this report. Appeal rights were provided. A Notice of Site visit was posted and must remain posted on or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
A Temporary Suspension Order Notice (LIC 9031) was posted on the front door.
