This is an amended report. On 11/28/2023, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation to deliver the findings for the above allegations. LPA met with Director Brittany Harris (D1). Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, and interviews. LPA reviewed and collected documentation pertaining to the allegations. It was alleged that staff members left a child unsupervised in a classroom. Interviews revealed that a child was left alone and unsupervised in a classroom on 10/31/2023 for approximately 10 minutes. It was alleged that staff yelled at children. Based on statements obtained during interviews, a staff member has raised their voice towards children.

Based on interviews, file reviews, and observations conducted the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Deficiencies were Cilcited on a subsequent 9099-D page.

On 11/28/2023, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation to deliver the findings for the above allegation. LPA met with Director Brittany Harris (D1). Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, and interviews. LPA reviewed and collected documentation pertaining to the allegation. It was alleged that staff handled a child in a rough manner. Interviews revealed conflicting information.

Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED. In the areas that were evaluated on 011/28/2023, no deficiencies were cited during today's inspection. An exit interview was conducted with D1, and Licensee Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

D1 acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099-D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. A Civil Penalty was assessed on a subsequent 9099-D Page.
