13 | On 03/10/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation. LPA met with Director Ashley Sanchez (D1). Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that a day care child was bullied by 2 other children while under the supervision of the facility. The video footage, interviews, and documents collected all reveal that the preschool staff did not provide visual observation of the children when T1 allowed 3 preschool children to enter a school age classroom. During this time, C1 had his/her head placed under water and held down by 2 other chidlren.
Based on interviews, documents, and observations conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Type-A and Type-B deficiency was cited on subsequent 9099-D page(s); these are repeat violations for the facilty and civil penalties will be assessed. D1 acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099-D with Type A deficiencies for 30 days and provide copies of this licensing
