25 | Application Specialist (AS) Alecia Sifuentes met with Licensee Representative, Rosa Silva-Tichauer for the purpose of an announced pre-licensing change of ownership inspection. Applicant requests a school-age license to serve 56 school-age children from kindergarten and above. The program will operate Monday through Friday from 6:30 a.m. to 6:00 p.m. The granted fire clearance was received on 8/26/2021.
Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, car seat poster, menus, and daily schedule. AS discussed the forms that must be in each child's and each staff member's file. AS provided LIC311A, Effects of Lead Exposure brochure, and Healthy Schools Act survey. The facility will be providing afternoon snack during the school year and morning snack, lunch, and afternoon snack during holidays and summer.
INDOOR ACTIVITY SPACE:
There are two school-age classrooms. AS observed a sufficient amount of equipment, tables, chairs, and cubbies. There is a first aid kit in the classroom. Medications will be stored in the office in a locked box. AS observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. AS observed a water pitcher and cups in the classroom. AS observed a functional carbon monoxide detector in all classrooms. AS observed an electronic sign-in/sign-out system.
