13 | n 02/12/2026, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation to deliver the findings for the above allegation. LPA arrived at the Family Child Care Home (FCCH) and disclosed the purpose of the visit. L1 granted entrance and LPA observed 2 infants, 1 preschooler, and 1 school aged child being supervised by L1 and her spouse. LPA accessed Guardian to determine that all required adults were background cleared and associated to the license.
Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that a day care child sustained an unexplained injury while in care on 01/12/2026. Licensee observed a red mark above an infant's ankle on 01/12/2026. Video Footage showed that Licensee notified the parent of the mark during pick-up of the infant on 01/12/2026. 2 LPAs were present at the FCCH on 01/12/2026 and did not observe any injury occur.
Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED. In the areas that were evaluated on 02/12/2026, no deficiencies were cited during today's inspection. An exit interview was conducted with Licensee Sultani and Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
