On 7/8/26, Licensing Program Analyst (LPA) Talwinder Bains arrived unannounced to deliver complaint findings for allegations listed above. LPA met with Administrator,Emanuel Dirar during today's visit and explained the purpose of the visit.
LPA Bains interviewed staff and residents during complaint investigation. Department investigated the stated allegation of this complaint. Department conducted a tour of the facility and conducted interviews with administrator, residents, and staff. Interviews did not indicate any residents, staff and/or witness observed that staff are threatening any residents in care. Department observed while doing facility tours that facility staff appeared to be attentive to resident’s needs and treating residents with dignity and respect. During residents’ interviews, residents stated that facility staff treat all residents with respect and dignity. Based on facility tours, interviews and observation, department found out that there is no evidence that resident (s) are being threatened by facility staff, therefore this allegation is found to be UNFOUNDED. A finding that the allegations are unfounded means that the allegations are false, could not have happened, and/or is without a reasonable basis.

On 7/8/26, Licensing Program Analyst (LPA) Talwinder Bains arrived unannounced to deliver complaint findings for allegations listed above. LPA met with Administrator,Emanuel Dirar during today's visit and explained the purpose of the visit.
The department conducted record review, interviewed staff and residents to investigate this allegation. Record review for medications administered logs (MAR ) for May and June 2026 indicated that staff were administering resident’s medications per their doctors’ orders and were keeping proper documentation. Residents interviewed indicated that staff were administering their medications as prescribed by their doctors. Staff interviewed reported that residents’ medications were administered and documented properly. Interviews with resident’s indicated staff administered tablespoon instead of teaspoon of Metamucil powder. There was insufficient information available to validate the concern however it was learnt that R1 now keep their own measurement spoon to ensure that staff are giving them the ordered dose of this medication. Based on gathered information, this allegation was Unsubstantiated. This agency has investigated this allegation, and it was concluded that it was Unsubstantiated.A finding that the complaint allegations is UNSUBSTANTIATED means that although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
