25 | Licensing Program Analyst (LPA) Anthony Tuck arrived unannounced at the facility on 01/07/2022 and was met by Executive Director (ED) Lupe Ramirez. LPA explained the purpose of the visit to conduct a case management visit.
LPA observed the facility to be following the mitigation plan for visitor entry and following new PIN guidelines for new screening process of visitors.
LPA learned that the facility was issued a citation on 03/24/2021 by CAL OSHA for not having staff FIT tested for use of N95 masks. LPA discussed prior incident with ED, LPA learned that the facility took corrective action by having all staff FIT tested. LPA learned that the facility continues to FIT test staff for N95 mask requirements to remain in compliance with CAL OSHA and California Health and Safety regulations. LPA learned that the facility was doing walk in appointments for FIT testing at Wellnesssmart, MD located at 558 La Sierra Drive Sacramento, CA 95864. LPA reviewed records of staff who were FIT tested on 10/09/2020, and 11/05/2020. LPA learned that the facility had staff who were scheduled to go to get FIT tested today 01/07/2022 for walk in appointments but was informed that the staff who were scheduled called out due to other reasons. ED stated that she also submitted an appointment request to Concentra to schedule staff for FIT testing and provided LPA with a screen print of the request to Concentra. LPA was informed by ED that the facility will have FIT testing for all staff completed by target date 02/28/2022.
Based upon observations, interview and documentation received, the facility was found to be following California Health and Safety Regulations. No citations or deficiencies were cited during today's case management visit.
Exit interview conducted with Lupe Ramirez, a copy of the report was left with the facility upon exit.
