25 | Licensing Program Analyst (LPA) Victoria Brown arrived unannounced to conduct a Case Management visit. Community Care Licensing (CCL) received notice that resident #1 (R1) was sent to diaylisis with unkept hygiene.
LPA interviewed Administrator, Caregiver, Transportation Driver, and staff at the Davita Florin Dialysis on 8/4/21.
On 7/22/21, R1 arrived to dialysis with poor hygiene such as feces on wheelchair and soiled pants. On 7/24/21, R1 arrived to dialysis disoriented, eyes closed and unable to respond. 911 was activated and R1 was diagnosed with Sepsis.
The investigation revealed that R1 preferred to manage personal hygiene, however, R1 was diagnosed with dementia and needed assistance with activities of daily living. The assistance was not provided prior to R1 going to appointment.
Per California Code of Regulations (CCRs) - Title 22, Division 6, Chapter 8, the following deficiencies are being cited on the attached 809D during this visit. If any of the cited deficiencies are not corrected by the noted due dates; civil penalties may be assessed. Susan McClure was provided a copy of their rights (LIC9058) and their signature on this form acknowledges receipt of these rights. An exit interview was conducted, and a copy of the report was given. |
